Changes to your home may be part of your care plan. These changes will: 

• Help you stay healthy and safe
• Help you move about your home and community

Money Follows the Person Program (MFP)

If your health gets better and you can leave a nursing facility, we’ll help you move back home. We will also cover the cost.

Community Transition Services

This is financial aid to help cover the cost of setting up your home when you move from a facility.
