Community Transition Services

This is financial aid to help cover the cost of setting up your home when you move from a facility. 

Things Paid for Include:

• Fees for your move
• Putting in ramps or grab bars
• Moving costs
• Furniture and household items
• One-time pest control and cleaning
• Security deposits
• Fees to set up electricity or phone

It Does Not Cover:

• Changes to a car or van
• TVs or other recreational items
• Rent/mortgage and utility bills

You can use this service once in your lifetime. The cost cannot go over $5,000.
